When House Calls Total Wellness founded Transition House in 2022, it was in response to the significant challenges faced by adults with mental health needs. Those key pain points include:

  • Limited Housing: Adults with mental health needs, both young and old, often struggle to find appropriate housing options.

  • Isolation and Social Deprivation: Individuals often experience a lack of motivation, leading to isolation.

  • Family Conflict: Living with family can cause conflicts and setbacks for those who need to become emancipated.

  • Inadequate Alternatives: Assisted living facilities for frail seniors are often the only option, and not suitable for young adults.

  • Long Waitlists: County-subsidized housing has long waiting lists, leaving individuals without immediate support.

Transition House addresses these needs by filling the gap between crisis centers, hospitals, and long-term housing solutions. Our approach includes:
